Student Dental Health Certificates

As a part of your child’s requirements for school, a physical examination has been required for new students and students in Kindergarten, Grades 2, 4, 7 and 10. A law was recently enacted that expands health screenings to include the dental health of students in New York State. 

Education Law and regulations of the Commissioner of Education have been amended in regard to dental health certificates.  Effective September 1, 2008, all public schools must request a dental health certificate from each student entering Kindergarten and Grades 2,4,7 and 10. 

When we require that your child have a physical examination, we will be requesting a dental certificate as well.  Click here to download a Dental Health Certificate to take to your child’s dentist.  Once completed, it should be returned to the School Nurse to be filed in your child’s Cumulative Health Record.
