HEALTH EXAMINATION/PHYSICAL REQUIREMENTS
For the school year 2022/2023
Physicals are required for all Pre-K, K, 1st, 3rd, 5th, 7th, 9th, and 11th-grade students and new enrollments. Physicals are also required to play sports and to receive a working card.
Please note that required physicals must be completed within 30 days of entering school. A copy of a physical performed within the past school year (after September 1, 2021) will meet the requirements.
Physicals will be offered at the Middle High School and/or Au Sable Forks Elementary on
Tuesday, May, 10th, Friday, May 13th, and Thursday, May 19th.
And at Keeseville Elementary Building on May 25th.
