Health Examination/Physical

For the school year 2023/2024

Physicals are required for all Pre-K, K, 1^st, 3^rd, 5^th, 7^th, 9^th, and 11^th grade students and new enrollments. Physicals are also required to play sports and receive working papers.

Please note that physicals must be completed within 30 days of entering school. A copy of a physical performed within the past school year (after September 1^st, 2022) will meet the requirements.

Physicals will be offered at the Middle High School and/or Au Sable Forks Elementary on May 11^th, 16^th, and 18^th.
