Please sign up in the Health Office at least one week before physical date if your child:
- Will be going into Grades Pre-K or K/New Entrants, 1, 3, 5, 7, 9 and 11.
- Will participate in a sport
- Will need working papers for employment
- New entrants
** Physicals are only valid for one year for any of the above reasons**

Cold and Flu season is here…but don’t forget respiratory etiquette! Help keep your families healthy!
Go to www.health.ny.gov/healthaz for reminders on respiratory illness prevention and influenza , as well as other conditions. Simply click on Health Topics A-Z- NYSDOH, then click on “P” for pertussis or “I” for Influenza.
Remember, wash your hands frequently, especially before touching your eyes, nose, or mouth, and always before eating!

NOTE:
*NYSED requires a physical exam for new entrants and students in Grades Pre-K or K, 1, 3, 5, 7, 9 & 11; annually for interscholastic sports; and working papers as needed. A law was recently enacted that expands health screenings to include the dental health of students in New York State.
Education Law and regulations of the Commissioner of Education have been amended in regard to dental health certificates. Effective September 1, 2008, all public schools must request a dental health certificate from each student entering Kindergarten and Grades 2, 4, 7 and 10.
When we require that your child have a physical examination, we will be requesting a dental certificate as well. Once completed, it should be returned to the School Nurse to be filed in your child’s Cumulative Health Record.
